EHR-MDS APIs - New Attribute at "Get Assessment By Id"

A new attribute response_text, which returns the text of the response to an assessment question, has been added to the response schema of the Get Assessment By Id endpoint. If "response_text" = null", then response_text may be omitted from the API response.
