EHR-MDS Conditions, Diagnostic Reports, Observation, and Progress Notes Endpoints

Five new API endpoints under Conditions, Diagnostic Reports, Observation, and Progress Notes and a HealthCheck endpoint were added for EHR-MDS integration.
